HC3 to be part of the '23 cycle of Healthcare Transformation Collaboratives | Update 10.10.2022

The Illinois Department of Healthcare and Family Services chose six health care collaboratives for the State Fiscal Year 2023 cycle of the Healthcare Transformation Collaboratives program, which aims to close gaps in health care services and eliminate the barriers to access and inequities that persist in Illinois’ health care system.


HC3 is proud to be part of the Developmental and Behavioral Health Pediatric Center (DBHPC) led by the Saint Anthony Hospital Foundation. As part of this collaboration, HC3's role will be to assist with strategically disseminating information and best practices, support research on health inequities in the Chicago market to complement the collaborative's goals, and manage a learning collaborative to advance change management strategies among project participants and beyond. We look forward to supporting this effort as part of our mission to harness our collective resources to drive meaningful change in our local community.
